Sam Shake - Chief Executive Officer
Sam became President/CEO of Biolife in October 2008.  
Prior to that date, he was Director of Operations and Technology  where he was responsible for leading the Manufacturing,
Logistics, Materials Management, Engineering and Research functions within the organization.
10 years prior to joining Biolife, Sam co-founded and was the Vice-President and Managing Partner of LEADERSHIFT
INTERNATIONAL INCORPORATED, an international leadership advisory firm focusing on the overlapping and interactive
elements of Leadership, Involvement and Improvement.  He was a member of the management seminar faculties of Michigan
State University and the University of North Florida.
Sam’s previous experience was with The Pillsbury Company.  He logged 25 years of direct experience in several functions,
including Research and Development, Quality/Regulatory and Operations.  Sam retired as the Director of Quality for Pillsbury’s
largest division.  Sam received his undergraduate degree from Indiana State University and his MBA from the University of
Saint Thomas.

Tim Kelly - Executive Vice President
To excel in his Biolife co-responsibilities for overall business growth and for building the distribution channels, Tim brings
an exceptional background in strategic sales management and sales accomplishments.  Tim was responsible for Becton
Dickinson's Canadian sales and for National Accounts in the United States.  Tim was also Vice President Sales for Friction
Free Technologies, where he successfully launched their line of low friction fibers to the sports, military and healthcare
markets.  This solid combination of established sales management and groundbreaking sales approach has been
instrumental in the exceptional trade support Biolife's products are receiving.  Tim has a Bachelor of Arts degree in Political
Science from Indiana University.

Andrew McFall - Vice President of Marketing
Andrew has over 15 years of experience with PepsiCo, Procter & Gamble and Andersen Consulting in building brands and
leading multi-functional teams to launch new products and marketing programs.  At Procter & Gamble, Andrew led the
strategic planning and execution for brands like Jif, Hawaiian Punch and Sunny Delight, and launched new products.  Most
recently, at Tropicana/PepsiCo, Andrew co-led Tropicana Pure Premium, a $1.7 billion business, and also managed a $150
million portfolio of brands including Dole and Tropicana product lines.  Andrew graduated magna cum laude with majors in
Economics and Mathematics from Vanderbilt University and received an M.B.A. from Duke University (Fuqua School of
Business).

Michele Slingluff - Chief Financial Officer
Michele's primary business experience prior to Biolife was as Controller with Teltronic, Inc. on both the operations side and
accounting side of the business.  She has an extensive and thorough background in corporate operations financial
management.  Michele was responsible for all aspects of the general ledger and financial statement preparation, budget
preparation, implementation and analysis.  Michele received a Bachelor of Science degree in Accounting and a Masters of Arts
in Public Administration, both issued by the University of Akron.  She holds a CPA license (inactive).

Claudia Masselink – Director of Quality
Claudia has over 15 years of experience in Quality Management (pharmaceutical and medical device industry). She brings an
extraordinary background in cGMPs and regulatory requirements. Prior to joining Biolife, Claudia was a Compliance
Coordinator for Hoffmann-La-Roche responsible for the systems that ensure product and process quality. At Biolife, her
responsibilities include ensuring maintenance and continuous improvement of our Quality Management System. She is also
responsible for ensuring all relevant quality requirements are met for products and processes while maintaining compliance
with US Department of Health and Human Services FDA Regulations, ISO13485, CMDCAS, and European Union Medical
Directive (MDD). Claudia has a Bachelor of Science in Biology from Montclair State University.

Kelly Keene – Director of Research & Development
Kelly brings extensive knowledge of ion-exchange resins from his seven years with Advanced Separation Technologies, a
subsidiary of Florida Progress. While at AST he participated in both plant design and start-ups. Kelly was the first employee of
Biolife in June of 2000. His contributions have been a major factor in converting a product concept into a working product. As
Director of R&D, He is responsible for all technical and development efforts at Biolife. Kelly has several issued and pending
patents. He graduated with a BS Chemistry from the University of South Florida.

John Hen, PhD – Technology Development Manager
John brings to Biolife more than 40 years of product and process experience and innovation, particularly in the field of
polymers. He commercialized numerous and diverse products with multi-million dollar sales at Mobil Oil, Uniroyal and
Engelhard. At Biolife, his responsibilities include clinical and preclinical studies, research in hemostasis, medical device
delivery, wound healing, antimicrobial activity and support of 510k submissions. He is the inventor of 17 issued U.S. patents
and 6 pending patents. He completed undergraduate chemical engineering studies at Mapua Institute of Technology, Masters
of Science at Utah State University and PhD studies at Rutgers University.

Louis M. Guzzi, MD, FCCM – Chief Medical Advisor
Doctor Guzzi completed his undergraduate work at Johns Hopkins University then graduated cum laude from Georgetown
University College of Medicine in 1986. He completed an Internal Medicine Residency at Eisenhower Army Medical Center
before an operational tour as an internist assigned to the 101st Airborne at Fort Campbell Kentucky. He furthered his training
at Walter Reed Army Medical Center by completing an Anesthesia Residency followed by a critical care fellowship. Later, he
became the director of Combat Trauma Research at the Walter Reed Army Institute of Research with a concentration in
combat casualty resuscitation and field management and transport. He authored the military chapter on transport of critical
combat casualties. He was awarded two ARCOM's and a Meritorious Service Medal for advancing battlefield care. After leaving
the military he started a critical care practice at Florida Hospital in Orlando Florida. He has served as Chair of Infection Control,
vascular access, Code 90 and Medication Safety and is a member of the IRB. He actively practices both anesthesia and
critical care medicine and lectures nationally and internationally on both topics. His current passion is in advancing the
decrease in medical errors and improvements in patient safety and infectious risk prevention strategies.

Roger Thomas - Strategic Support Projects
Roger brings to Biolife a wealth of management and business development experience and creativity stemming from more
than 40 years of experience leading manufacturing operations with major companies, including General Electric, Whirlpool,
Borg Warner, Converse, and Berol Corporation.  Roger has engineering and business administration degrees from Marshall
University.
